If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. For your convenience we accept Visa, MasterCard and Discover. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
Our relationship is with you our patient. As a courtesy to you we accept and file most insurances. We accept an “assignment of benefits” from you which means that the insurance company will pay us directly for any covered services. You will be responsible for any estimated “out of pocket” expenses and or deductibles at each visit. You may or may not have insurance coverage for certain Dental treatment that you may need or want. Most insurance companies will respond within three to four weeks. If after 30 days we have not received payment from your insurance company you will need to pay any outstanding balance and contact your insurance company directly. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance to any outstanding balances is appreciated.
We are providers with Anthem, Cigna, Delta Dental, all carriers under the Dentemax network, Dentequest-Smiles, Guardian, Principal, and United Concordia. This means that we have a special relationship with each of these companies that will save you money when you visit our office. We accept most Dental insurances and will file your claims as a courtesy to you. At Spring Creek Family Dentistry we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. Please call if you have any questions or concerns regarding your initial visit. You can expect to pay your “estimated” portion for all procedures at each Dental visit. We do the best we can in providing you with the closest “estimate” of your financial responsibility. Many times there is a remaining balance for Dental procedures you have had completed after the insurance company has paid on your behalf. If there is any outstanding balance due after your insurance company has paid we respectively request that you pay this portion within 7-14 days.
Please bring your insurance information with you on your initial visit to our office so that we can expedite reimbursement. Also, please provide us with all updated insurance information each year at your renewal date. Most insurance companies will issue you a new insurance card each year after your renewal period. Some employers may change insurance companies from one year to the next therefore, it is important that we have your updated information in order to expedite all of your individual claims.
