Insurance Information

We are providers with Anthem, Cigna, Delta Dental, all carriers under the Dentemax network, Dentequest-Smiles, Guardian, Principal, and United Concordia. This means that we have a special relationship with each of these companies that will save you money when you visit our office. We accept most Dental insurances and will file your claims as a courtesy to you. At Spring Creek Family Dentistry we make every effort to provide you with the finest care and the most convenient financial options. To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. Please call if you have any questions or concerns regarding your initial visit. You can expect to pay your “estimated” portion for all procedures at each Dental visit. We do the best we can in providing you with the closest “estimate” of your financial responsibility. Many times there is a remaining balance for Dental procedures you have had completed after the insurance company has paid on your behalf. If there is any outstanding balance due after your insurance company has paid we respectively request that you pay this portion within 7-14 days.

Please bring your insurance information with you on your initial visit to our office so that we can expedite reimbursement. Also, please provide us with all updated insurance information each year at your renewal date. Most insurance companies will issue you a new insurance card each year after your renewal period. Some employers may change insurance companies from one year to the next therefore, it is important that we have your updated information in order to expedite all of your individual claims.
